Benet Academy Athletic Permission Form

This form must be turned in prior to the individual partaking in any practice or tryout. The form should be handed directly to the head coach of the respective team and level of competition that the individual is eligible for. Please be advised that no individual in any sport will be allowed to participate until this form has been returned.

It should also be noted that physicals are required before any individual is allowed to physically participate in any interscholastic activity. This includes trying-out for any sport that will have team size limitations.
