If you are trying out for a winter sport, you must submit your registration forms to the athletic office at West Campus or the main office at East Campus.  Upon receipt of all the required forms, you will receive a receipt which must be given to the coach on the first day of tryouts.  Required forms are:

• Athletic Emergency Card (required for each sport).
• Lancer Code of Conduct (once in four years of high school).
• IHSA Consent to Random Testing (once in four years of high school).
• Current physical (less than a year old)
