"2008-09 IHSA *NEW* Physical form"
Here is a link to the 2009-010 IHSA Preparticipation Examination Form:

We prefer that if your son/daughter has not had their sports physical yet that this form be filled out by the doctor's office.
However, if you have already had their sports physical completed for next year and the doctor's office used an older form it will be still be accepted this year.
