WINTER SEASON PARTICIPATION REQUIREMENTS:
All students wishing to participate in a Winter sport must have their (4) required forms of paperwork turned into the Athletic office using the date guidelines below:
*11/2/09 - Season begins for:
Girls Basketball
Paperwork due by 10/26/09
*11/9/09 - Season begins for:
Bowling
Girls Gymnastics
Boys Basketball
Boys Wrestling
Paperwork is due by 11/2/09
*11/23/09 - Season begins for:
Boys Swimming
Paperwork is due by 11/16/09
Required paperwork is as follows:
(1) Medical Emergency card
(2) Parent Permission form
(3) IHSA Steroid authorization form
(4) Current physical (within 1 year)
